Get your Flu Shot at HQ on October 5th between 10:00 a.m. - 2:00 p.m.

 

Getting an Influenza vaccine is one of the best ways to avoid getting the flu.  In order to protect our staff, FCPL is pleased to announce that we have arranged for Walgreens to offer a flu vaccine clinic on Tuesday, October 5^th.  There will be one clinic from 10:00 am – 2:00 pm at HQ/Administrative Offices.

Your participation is completely voluntary, however if you choose to participate it will be considered work time and you may notate up to one hour on your time sheet (if you were not scheduled to work).  For those employees scheduled to work that day at one of the branches, please speak to your supervisor to arrange the best time to go.

We are also very pleased to be able to offer to cover the cost of the vaccine to any employee who does not have health insurance.  

What you need to do........

1. Email Elise Herbst herbste@forsythpl.org to let Elise know you want to sign up to get a shot on Oct. 5th
2. Tell Elise what time you want to sign-up to get your shot between 10:00 am - 2:00 pm 
3. Complete the insurance form (if applicable) that Elise will email to you
4. If you have insurance - bring your completed form with you when you come to HQ to get your shot
