Open Enrollment Checklist

Open Enrollment for the 2022 Plan Year begins today, October 18, 2021 and ends November 5, 2021.  Staff regularly scheduled to work 25 hours per week are eligible for benefits with GaBreeze. Staff regularly scheduled to work 30 hours or more per week are eligible for all benefits including health insurance with SHBP.

The following OE checklist will help you stay on track and make the 2022 benefit election process easy!

1. Refer to the Open Enrollment memo that was emailed to eligible staff on Friday, October 15^th.
2. Review the decision guides*, rate charts, and other plan materials posted to the 2022 Benefits folder on Sharepoint
3. Login to GaBreeze and/or State Health websites [First time SHBP users should use employer code: SHBP-GA to create their account.]
4. Follow the prompts to enroll/drop/change your 2022 benefits
5. Update/change your beneficiary designations on the GaBreeze website
6. Confirm your changes and complete your enrollment
7. Review and print your confirmation pages – send copies to HR

If you do not wish to make changes to your current 2021 benefit plan enrollments and you are not enrolling in a 2022 Flexible Spending Account, please email HR so that we can take you off our reminder list.
